Lower back pain (LBP) is a health issue that affects millions of people worldwide. In 2020, it was estimated that 619 million people suffered from LBP, and this figure is expected to rise to 843 million by 2050, primarily driven by population growth and aging. This condition is the leading cause of disability globally and represents the greatest potential benefit from rehabilitation for individuals. LBP can occur at any age, and it is estimated that most people will experience it at least once in their lifetime. Its prevalence increases with age, peaking between 50 and 55 years, with women being more commonly affected.

An Overview of Lower Back Pain

LBP is defined as pain that occurs between the lower edge of the ribs and the buttocks. It can be acute, lasting less than six weeks; subacute, lasting between six and twelve weeks; or chronic, persisting for more than twelve weeks. This type of pain not only hinders movement but can also affect quality of life and mental well-being, limiting work activities and interaction with family and friends. In terms of classification, LBP can be specific, associated with a disease or structural problem, or non-specific, where a clear cause cannot be identified, representing around 90% of cases. In this context, it is essential to understand how pain is an individual experience shaped throughout a person’s life, influenced by psychological, physical, and social factors.

The Magnitude of the Problem

LBP is one of the most prevalent musculoskeletal conditions globally and represents a significant public health problem. Currently, 619 million people live with this pain, and its impact on quality of life is notable. Restrictions in social and work participation are common, and many experience recurrent episodes as they age. The economic burden associated with LBP is significant, as it affects workplace productivity and incurs costs for both individuals and entire societies. Additionally, it has been documented that rates of depression and anxiety are considerably higher in people with chronic LBP (cLBP), suggesting that LBP can create a difficult cycle to break, where reduced physical activity leads to maladaptive beliefs about pain, such as catastrophizing and kinesiophobia.

Associated Symptoms and Signs

LBP can present as a dull or sharp pain and may radiate to other areas of the body, especially the legs. This restriction of movement can affect not only work and academic performance but also social and familial relationships. Furthermore, it can lead to sleep problems, as well as low mood and emotional distress. In some cases, radicular pain, known as sciatica, may accompany LBP, characterized by tingling sensations or weakness in the legs. The combination of these symptoms can contribute to a significant deterioration in the quality of life for those affected.

Causes and Risk Factors

The classification of LBP into specific and non-specific types is crucial for treatment. The World Health Organization (WHO) is taking steps to expand access to care for individuals with LBP through various initiatives. The Rehabilitation 2030 Initiative offers a package of rehabilitation interventions, including assistive products and resources for health conditions like LBP. The Decade of Healthy Aging recommends a reorientation of health systems and care to promote healthy aging and address the diverse needs of older adults. The Integrated Care for Older People (ICOPE) approach promotes person-centered assessment to design personalized care and prevent the loss of locomotor and psychological capacity due to pain. Finally, the WHO’s guidelines on the management of chronic low back pain, currently in development, will provide evidence-based recommendations on non-surgical care in primary and community care settings.
